First Visit
Your initial appointment usually will consist of an examination, x-rays, and review of your treatment options. Dr. Malan will personally present to you your current oral health status, and make recommendations for a treatment plan specifically tailored to your needs and desires. This is an excellent opportunity to ask any questions about your treatment options and have them answered directly by Dr. Malan. Our office will then schedule any required follow-up appointments for teeth cleaning and restorative work.
Please assist us by providing the following information at the time of your consultation:
- Any x-rays taken by a previous dentist, if applicable.
- A list of medications you are presently taking.
- If you have dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
X-Rays
If your previous dentist has taken x-rays, you may request that they forward them to our office. Since our office uses the latest in digital x-rays, your previous dentist’s office may email us any copies if they are also in digital format.
If your previous x-rays were of the traditional film type, obtaining the originals rather than copies is most beneficial. If there is not enough time to have them mailed to us, please pick them up and bring them to our office. In any case, all previous x-rays must be complete and of acceptable diagnostic quality for Dr. Malan to provide you with an accurate evaluation. If additional x-rays are necessary, they can be taken at our facility.
