Patient Registration
If you are a new patient, you may complete the registration forms electronically or print them and bring them with you to your first appointment.
Please review the Notice of Privacy Practices and the Dental Materials Fact Sheet by clicking the links provided. By law, our office is required to make this information available to patients prior to performing any restorative dental treatment. The California State Dental Board last updated the Dental Materials Fact Sheet on May 14, 2004.
